Permit Forms On-Line

You can now download certain permit application forms required for submission to the Division of Building & Housing. After you've printed the forms, review the submission requirements and the fee schedules. When you've completed the permit application, bring it and your check (payable to City of Cleveland) to Room 505, City Hall, 601 Lakeside Avenue, Cleveland, Ohio 44114 for approval and processing.

Not sure what forms you need?

You need a "Plumbing, Sewer and Gas Piping Systems Application for Permit" if you are:

• New, extended or repaired water service
• New, extended or repaired sewer lines
• New, extended or repaired gas service
• New, extended or repaired plumbing systems
• New or replaced plumbing fixtures
• New or replaced water heaters
• Power-assisted sewer cleaning
• Correct violations

You need a "Heating, Ventilation, Air Conditioning/Refrigeration Application for Permit" if you are:

• New, extended or repaired ductwork
• New, extended or repaired hydronic lines
• New, extended or repaired control wiring
• New or replaced HVAC equipment (Furnace, Boiler, Unit heaters, etc.)
• New or replaced Refrigeration equipment
• Correct violations

You need an "Electrical Wiring and Systems Application for Permit" if you are:

• New, extended or repaired electrical service
• New, extended or repaired low-voltage fire alarm system
• New, extended or repaired temperature control wiring
• New temporary electric service
• New electrical fixtures, equipment or devices
• New or repaired illuminated sign connections
• Blanket electrical permit for commercial premises
• Correct Violations

You need an "Affidavit for Home Owner's Permit" if you are:

• Owner/Occupier of single or two-family residences can obtain permits for performing work on their properties after filing this form. In all other cases, a City of Cleveland registered contractor must obtain the permit and perform the work.
